Your New-Patient Paperwork, Start to Finish

Demo content — Northline Family Dental is a fictional practice created to demonstrate this website template. The staff, hours, phone numbers, prices, and policies below are made up. Nothing here is medical or dental advice.Paperwork is the part of a first appointment most likely to go sideways. It takes about ten minutes if you do it at the kitchen table, and about twenty-five if you do it in the waiting room with a pen that does not work.
What we send you
Three short forms, by email or through the portal, as soon as you book:
- Contact and responsible party. Who you are, how to reach you, and who is responsible for the account — which matters most for children and for anyone booking on someone else's behalf.
- Contact preferences. How you want reminders: text, email, phone, or none. You can change this any time.
- Payment and billing policy. What we charge for, when payment is due, and what happens if an appointment is missed. Read this one properly. It is the form people skim and then have questions about later.
What we do not ask you to type online
Our web forms do not ask for health history, medications, plan ID numbers, or your date of birth — and you should not add them to a free-text box either.
That is deliberate. A website form is fine for a phone number and a signature. It is not the right place for health information. Anything of that kind is handled on paper at the front desk or by phone, where it belongs.
If a form ever seems to be asking you for medical detail, stop and call us. Something has gone wrong on our end.
Three ways to get the forms back
- Upload them to the portal as a PDF or a photo of each page
- Bring them printed, signed, on the day
- Come in fifteen minutes early and do them here on a clipboard
All three are equally fine. Nobody gets a better appointment for choosing one over another.
What holds paperwork up
Nearly every delayed check-in comes down to one of four things:
- A missing signature on the last page
- The responsible-party section left blank on a child's form
- A phone number we cannot read
- An old address, so the reminders went somewhere else
A thirty-second check before you send saves the whole thing.
Reading the estimate you get back
If the dentist recommends anything beyond an exam and cleaning, you get a written estimate before it is scheduled. It has four columns:

Example from a fictional estimate: a filling at $240, with an estimated plan portion of $150 and an estimated $90 for the patient. Your practice, your plan, and your treatment will all produce different numbers.
Ask for a printed copy before you leave, and ask us to go through it line by line. That is a normal request and takes two minutes.

Billing and paperwork questions go to the front desk — call or message us, or send them through the portal. For anything about the treatment itself, ask the dentist directly. This guide explains the forms, not your care.
